Our Prevention Services are designed to provide targeted, whole-person care and deliver proactive support to individuals and families, helping them navigate challenges before they escalate. We connect you with essential resources and personalized care right in your community, focusing on well-being, stability, and long-term health.
Our Prevention Programs offer a collaborative and whole-person approach to health and well-being. We partner with you to identify your unique needs and build a personalized path to success.
Populations of Focus (POFs)
Our services are specifically tailored to support individuals and families who have the most complex needs. We currently serve the following populations:
- Individuals Experiencing Homelessness
- Individuals At Risk for Avoidable Hospital or Emergency Department (ED) Utilization
- Individuals with Serious Mental Health and/or Substance Use Disorder (SUD) Needs
- Adults Living in the Community and At Risk for Long-Term Care (LTC) Institutionalization
- Children & Youth Enrolled in California Children’s Services (CCS) or CCS Whole Child Model (WCM) with Additional Needs Beyond the CCS Condition
- Children & Youth Involved in Child Welfare
- Individuals with Intellectual and Developmental Disabilities (I/DD)
- Pregnant and Postpartum Individuals (including the Birth Equity Population of Focus)

Enhanced Care Management (ECM)
Enhanced Care Management (ECM) is a hands-on, comprehensive approach to whole-person care, ensuring your healthcare and social services work together seamlessly.
We help you with:
Accessing Essential Services: We help you find primary care doctors, dentists, mental health therapists, and specialists.
Care Coordination & Navigation: We coordinate various health and social services and help you navigate complex health systems (including scheduling appointments, arranging transportation, and facilitating follow-up care).
Addressing Treatment Barriers: We identify and resolve barriers to treatment adherence, such as lack of transportation, advocating on your behalf, and ensuring clear guidance on prescribed medications.
Unified Care Planning: We create a comprehensive, unified care plan that addresses your physical, behavioral, and social needs for long-term health and stability.
Connecting to Community Resources: We link you with vital community resources, including housing support, food assistance, and local programs.
We partner with a wide range of Managed Care Plans to deliver Enhanced Care Management (ECM) services across California.

Community Supports (CS)
Addressing Social Needs that Impact Health
Community Supports (CS) provides non-medical services that address the social and environmental factors that directly affect your health. These essential supports help remove barriers and promote a stable, healthy living environment. Our Community Supports focus on housing navigation (including assistance in searching and applying for housing) and housing tenancy and sustaining services to ensure stable housing.
Housing Supports (Housing Trio) services include:
- Housing Transition Navigation Services
- Housing Deposits
- Housing Tenancy & Sustaining Services
CS Eligibility Criteria
- Must be an enrolled Kaiser Permanente Medi-Cal member.
- Must meet criteria for one or more of the Community Supports established by the Department of Health Care Services (DHCS).
- Members receiving Enhanced Care Management (ECM) or Community Health Worker (CHW) services can be eligible for CS, as can members not receiving these services.
- Kaiser Permanente determines eligibility and authorizes services.
Community Health Worker (CHW)
Trusted Guidance, Right in Your Neighborhood
Our Community Health Workers (CHWs) are trusted members of the community who serve as a link between health and social services and the families who need them. They provide culturally relevant information, guidance, and support to empower you to take control of your health.
CHW services focus on prevention and support. We help prevent disease and disability, promote physical and mental health, and improve quality of life. For members with Medi-Cal, we can provide these services and more to ensure you have the resources and guidance needed to stay healthy and supported.
CHWs can help by:
- Providing health education on topics like chronic disease management and preventive care.
- Advocating for your needs within the healthcare system.
- Connecting you with local resources and support groups.
CHW Eligibility Criteria
- Must be an enrolled Kaiser Permanente Medi-Cal member.

Healthy Families America® (HFA®)
Early Support for Families, From the Very Beginning
Healthy Families America® (HFA®) is a voluntary, no-cost home visiting program that supports expectant parents and families with young children during pregnancy and early childhood. As part of Koinonia Family Services’ Prevention Services, HFA® helps strengthen parent-child relationships and support healthy development before challenges escalate.
Families partner with a trained Family Support Specialist who provides parenting support, encouragement, and connections to community resources—guided by each family’s goals and needs.
Currently available through our Fresno, Visalia, and Modesto offices.
HFA® Eligibility Criteria
- Pregnant individuals in their third trimester.
- Parents or caregivers of an infant age 0-3 months.
- Foster parents or families with a child age 2 or younger (active child welfare case).
- Residents within the program’s service area.
- Interested in participating in a home visiting program for 3-5 years.
- Available for regular (typically weekly) home visits at their primary residence.

HFA® is designed for individuals and families who meet the following criteria:

Interested in participating in a home visiting program for 3–5 years

Available for regular (typically weekly) home visits at their primary residence
Participation is always voluntary, and a Koinonia staff member will follow up to discuss next steps.
